Other personal information collected as part of providing the psychology service
Collection, Storage and Use of Your Personal Data
Your information is collected before and during consultations. Client records are stored in a secure electronic Australian practice management system and/or a secure filing cabinet, accessible only to Dr Katie Trickey, and, in the case of non-clinical material, to authorised administrative staff. Your psychologist is responsible for maintaining the privacy of your session notes.

The Company may use Personal Data for the following purposes:

To provide and document a psychology service, your psychologist will only record information deemed necessary or relevant to provide your treatment and inform opinions that may be requested, such as a letter to your GP regarding your treatment and progress.

To contact You: To contact You by email, telephone calls, SMS, or other equivalent forms of electronic communication, such as a mobile application's push notifications regarding updates or informative communications related to the functionalities, or contracted services, including practice updates, and appointment reminders.

To manage Your requests: To attend and manage Your requests to Us.
Disclosure of Personal Information
Your personal information will note be disclosed except when We may share Your personal information in the following situations:

We may disclose Your personal information for any purpose with Your consent. For instance to provide a letter or report to another professional, eg. GP, doctor, psychiatrist, agency, lawyer. With your consent, and where this is deemed to be in your best interests, we may disclose your personal information to another person, eg. family member.

If subpoenaed by a court, or disclosure is otherwise required by law

Where failure to disclose the information would, in the reasonable belief of Dr Katie Trickey, place you or another person at serious risk of harm

Retention of Your Personal Data
Psychologists are required to retain adult client files for 7 years. In accordance with this requirement, we store client records securely for this time. Any information that cannot be stored electronically is archived in a secure filing cabinet. Once your record is due to be disposed of, your electronic file will be permanently deleted and any paper file will be shredded.
The Company will retain Your Personal Data only for as long as is necessary for the purposes set out in this Privacy Policy. We will retain and use Your Personal Data to the extent necessary to comply with our legal obligations (for example, if we are required to retain your data to comply with applicable laws), resolve disputes, and enforce our legal agreements and policies.
Requests for access to client information
Other than in the case of subpoenas by a court, requests to release your information must be received within at least 28 days prior to the information being needed. Your psychologist will require you to provide fully informed written to consent to release this information.
Dr. Katie Trickey will usually require you to attend an appointment to review the information due to be released and obtain your fully informed written consent.
Costs and time frames for these requests are at the discretion of Dr. Katie Trickey.
Please note we be responsible for preserving the confidentiality of information that has left our control.
Requests for correction or access to your information
You have the right to request to see your personal information by writing to your psychologist. Access may or may not be granted. Where access is granted you will be required to attend an appointment to discuss the information. Standard service fees apply.
You can may also request to correct personal information on your client record that is agreed to be inaccurate.
Please note, however, that We may need to retain certain information when we have a legal obligation or lawful basis to do so.
Professional Supervision
It is good clinical practice for clinical psychologists, however well-qualified, to receive clinical supervision from another senior psychologist, for the purposes of guidance, monitoring and support, and in order to uphold the highest professional and ethical standards. In such instances, your information would be de-identified by using a pseudonym and by removing all identifiable data.
Security of Your Personal Data
The security of Your Personal Data is important to Us, but remember that no method of transmission over the Internet, or method of electronic storage is 100% secure. While We strive to use commercially acceptable means to protect Your Personal Data, We cannot guarantee its absolute security.
